Blended Case Management Services

Blended Case Management is available through Sarah A. Reed Outpatient Behavioral Health.  The Blended Case Management Program is designed to assist the child or adolescent and his/her family in gaining access to services, monitoring the delivery of those services, assisting with problem resolution, and facilitating the linkage of client care with community resources.  The Blended Case Manager (BCM) works with the client and family to ensure that they are receiving the most appropriate and culturally competent services they require.  Through collaborative relationships with other service providers, the BCM assists the client and his/her family in gaining access to services and community resources to achieve stability in the community.

Sarah A. Reed Children’s Center offers Blended Case Management to children and adolescents of Erie County between the ages of 2.5 and 17 years of age (21 years of age, if enrolled in Special Educational Services).  Clients will need to have a mental health diagnosis and be receiving (or in need of receiving) mental health services and/or services from two or more human service agencies.

In order to create a child-centered, family-focused team, we are committed to modeling the Seven Commitments of the Sanctuary® Model: a commitment to Nonviolence, Emotional Intelligence, Social Learning, Shared Governance, Open Communication, Social Responsibility, and Growth and Change.  Using these principles as well as the S.E.L.F. model (Safety, Emotions, Loss and Future) the BCM will perform the role of working in concert with the client and family to support successful attainment of identified goals and objectives as a path towards improved functioning and a greater sense of resiliency, recovery, and competence.  The BCM will work to ensure that every client’s history of adversity is recognized in the delivery of their services as well as providing direct interventions to address social, emotional, physical and behavioral functioning.
